Who changed what, and why. Today the only audit stream in Setpoint belongs to the point-of-sale app, its writer swallows its own errors, and nothing records an academy override at all. New here: an append-only academy log with the actor, a required reason, the before and after values, and where the change came from. There is no primary action — the screen is read-only, and rows can never be edited or deleted.

A reason is required before the override is accepted, so the log never carries a blank explanation. If the log row cannot be written the override is refused — it is part of the same transaction, not a best-effort note.
